Hybrid brystforstørrelse - brystimplantat og fettransplantasjon
Noen kvinner opplever en forstyrrelse i utviklingen av brystene som fører til underutviklede, og/eller asymmetriske former på brystene. Andre har opplevd tap av fett og hengende bryster etter gjennomgått svangerskap og/eller vektreduksjon. Dette kan oppleves som både kosmetisk og intimt sjenerende for noen. Disse gruppene er vanlige kandidater til brystforstørrelse med brystimplantater og/eller fettinjeksjoner.
En økende trend er å kombinere innsettelse av implantat i kombinasjon med fett for dem som ønsker volumøkning i brystene med den naturlige følelsen fett har. Prosedyren egner seg for de som ikke har nok fett å flytte/bruke og for de som ønsker figurforming til andre områder på kroppen. Den naturlige looken ser man spesielt ved kløften og den perifere delen av brystet, altså den øvre delen, utsiden og under brystet.
Siden 2008 har Oslo Plastikkirurgi utført brystforstørrelse med eget fettvev, og vi har operert over 250 kvinner. Vi opprettet dette tilbudet som et alternativ til brystforstørrelse med implantater. Oslo Plastikkirurgi har derfor lang oppfølgingstid av disse operasjonspasientene, som har vist seg å være godt fornøyde med inngrepet sitt, og at fettvolumet i brystene har holdt seg stabilt.

Selve prosedyren foregår slik at det forberedes en lomme under brystkjertelen, en annen metode er under brystmuskelen. Et tredje og gjerne det beste alternativet, er å sette implantatet under hinnen til brystmuskelen (pektorales muskelen), da beskytter protesen med selve fascie (hinnen rundt muskelen) og man ungår samtidig å bruke selve muskelen.
En silikonprotese blir lagt inn i lommen av en størrelse og type valgt før operasjonen. Arret legges enten under brystet (mest vanlig), rundt brystvorten eller ved armhulen. En unngår rippling (rynker) av huden om implantatet settes under muskelen eller under fascie. Valget tas ut ifra hva som passer pasientens anatomi og vil gi det beste estetiske resultat.
Ammeevnen ødelegges normalt sett ikke av operasjonen eller muligheten til mammografiundersøkelse. Brystforstørrelsen øker heller ikke forekomsten av brystkreft. (se mer under FAQ)
Oslo Plastikkirurgi bruker Mentors proteser Memorygel som er brystimplantater fylt med Mentors patenterte kohesive gel som har en fast form, samtidig som den ligner den naturlige elastisiteten i brystvevet. Denne finnes i flere profiler, fra moderat til ultra høy profil. Mentor gir livstidsgaranti mot ruptur.

Mentor tilbyr også andre typer proteser som består av saltvann, eller en kombinasjon med saltvann og silikon. Protesens ytre inneholder silikon. Angående formen, kan protesene være runde eller dråpeformede, sistnevnte gir hos noen kvinner en mer naturlig form av brystforstørringen, men egner seg noe mindre til kvinner som ønsker fylde i øvre polen av brystet.
Om fett til bryst
Ved brystforstørring med eget fettvev forberedes små tunneler under brystkjertelen, alternativt i brystmuskelen der fettet bygges i lag uten trykk. Fettet tas ut og behandles på en spesiell måte før det injiseres med en spesiell kanyle i brystområdet, såkalt Colemans teknikk eller som vi nylig har begynt med; en vibrasjonsteknikk med MicroAire maskin som skaper mer plass til fettet i brystet, og stimulerer tilheling og bedre hold på fettet og dermed bedre resultat. Fettmengden som trengs til prosedyren, avhenger av det ønskede volumet i brystene. Ved en standard fettinjeksjon får man små arr på ca. 1-2 mm.
Resultatet av denne type kirurgisk teknikk avhenger av resorpsjon av det injiserte fettet, altså i hvilken grad kroppen selv suger bort fettet. Vanligvis innlemmes (gror) fettet godt i det mottagende området. Man må regne med at kroppen absorberer opp til 40 % av fettmengden de første månedene, derfor fyller man litt mer enn det man trenger (overkorrigerer) for å kompensere for dette svinnet. Man har heller ingen garanti for at det hos noen blir absorbert mer enn 40 %.
Håndtering av fettet er avgjørende for at det skal gro fast. Det eksisterer flere motsigende teknikker for å behandle fettet som fjernes, slik at kun rent fett blir injisert. Fettet må behandles varsomt uten stor skade på fettcellene. En sentrifugeringsprosess der man med en enkel maskin separerer fettet anses å oppfylle disse kravene. Vi bruker en lav grad av sentrifugering for å separere fettet, men beholder den fettvæsken som forsørger fettcellene med blant annet de naturlige tilvekstfaktorer og stamceller som vi synes gir et best mulig resultat.
Det er ikke uvanlig at det er behov for ny injeksjon, og ved resorpsjon på mer enn 40 % tilbys ny fettinjeksjon innenfor garantitiden; dette under forutsetning at det er tilstrekkelig fett til injeksjon for noe volumøkning eller i de tilfeller hvor asymmetri inntreffer. Hvis fettvevet ikke suges bort fra kroppen, slik at volumet ikke blir tilfredsstillende, forblir dette et naturlig og langvarig resultat.
Vi lukker såret med sting som løser seg opp av seg selv. I svært få tilfeller legges det inn dren (rør) som fjerner blod og væske fra sårområdet under brystet. Disse fjernes vanligvis dagen etter brystforstørringen. Brystforstørring gir i prinsippet ikke grunnlag for sykemelding. Fysisk trening og hardt kroppslig arbeid bør man vente med i ca. 4 uker. Arrene er i begynnelsen ofte røde, kløende og harde, men med tiden blekner og mykner de. Arrene skjules imidlertid av BH eller badedrakt og vil ikke synes i utringede kjoler. I sjeldne tilfeller blir arrene brede og godt synlige fordi dette er avhengig av kvinnens egne nedarvede egenskaper. Vi anbefaler derfor at du teiper arrene i et par måneder etter en brystforstørring.
Pick-up and possible accommodation
You must arrange in advance with someone to pick you up from the clinic approximately 1 hour after the surgery, as you cannot drive, take public transport or taxis alone. You must also have someone with you on the same day and the first night.
If you live far from the clinic, we recommend that you stay overnight in Oslo, either with family/friends or in a hotel.

Before the operation
You must be healthy on the day of the operation. If you are in doubt whether you have a cold or similar, you must contact us. The operation is not performed when you have the flu with fever, cough or other infection in the body. Before the operation you will receive a prescription for antibiotics, possibly painkillers and Hibiscrub solution 40 mg/ml which you can buy at the pharmacy.
The day before surgery and on the day of surgery, you should wash your entire body with Hibiscrub solution in the shower. This is a disinfectant soap that should be rinsed off with water, and the skin should be dried with a clean towel. Put on clean clothes when you leave home. Do not use perfume or cream. All jewelry, piercings and nail polish/false nails must be removed at the latest the day before surgery and may be put back on 7 to 10 days after surgery.
To get the best possible result and to avoid complications, it is recommended that you have a normal weight, and not an excessively high BMI. If you smoke, or use nicotine such as snus or electronic cigarettes, you must quit at least 2 weeks before the surgery and 2 weeks after the surgery.
You should arrive at the surgery fasting. This means that you should not eat food including fruit or drink juice with pulp or dairy products 6 hours before the surgery. You can drink water, juice, juice without pulp or coffee/tea without dairy products or chew gum/drops/throat lozenges up to 4 hours before the surgery. Medications that you may need to take on the day of surgery can be swallowed with 1 glass of water max. 100 ml up to 1 hour before the start of anesthesia.
It is important that you stop taking blood-thinning medications about 14 days before the operation, preferably after consulting your GP or us. For other medications, consult the clinic before the operation. A lot of garlic and health food preparations should also not be consumed.

On the day of surgery
You will be received by our staff who will prepare you for the operation before the surgeon sees you. Before the actual procedure, the surgeon will go over the procedure, draw up the plan, and answer any questions you may have. Anesthesia personnel will also come to greet you and answer your questions.
When the surgery is about to begin, the operating room nurse will pick you up and clean the surgical area while the anesthesia personnel will give you a sedative in your arm so that you are asleep during the surgery. In addition, the surgeon will apply local anesthesia to the surgical area for a long-lasting pain relief effect in the surgical area after the surgery, which will also reduce bleeding in the area.
Etter operasjonen blir du flyttet til oppvåkningsrommet der du har din egen seng. Du hviler så i 1-2 timer og får servert mat og rikelig med drikke i form av brødmat, yoghurt og saft (husk å informere om allergier). Evt smertestillende gis ved behov. Kirurgen ser til deg før hjemreise/ hotelloppholdet og du blir avklart av anestesipersonell før du reiser hjem med en voksen ledsager. PVK som er festet til hånden blir fjernet før du drar. Det må brukes bil eller drosje som transportmiddel rett etter operasjon, vi anbefaler ikke kollektivtransport eller det å gå.
After the operation
All patients will come for a post-operative check-up the day after surgery. We recommend that you do not travel by air for the first 48 hours after surgery. During this time, a check-up appointment will be arranged at the clinic and you will come for a check-up at the clinic the day after surgery.
The first few hours after the operation you may feel a little tired, and sometimes a little nauseous. It is important that you take painkillers as prescribed and as needed, and make sure to drink plenty of fluids during the first 24 hours. Some people may feel a little dizzy, so it is a good idea to take a few minutes to get up. For the first two days you should rest and lie as still as possible, with your upper body elevated. For the next five days you can move around carefully. If you have an active job, it is a good idea to stay home.
Etter operasjonen får du en spesial-bandasje som fjernes etter 3 dager, teipen beholdes på i 2 uker. Du skal bruke sports-BH i 6 uker etter at bandasjen er fjernet. Etter at teipen fjernes (etter 2 uker) kan du dusje området og sette på ny teip. Det anbefales at du teiper arrene med hudvennlig teip i minst 2 måneder. Kirurgisk teip som Micropore eller Steri-Strip kjøpes på apoteket, alternativt silikon teip.
Vi lukker såret med sting som løser seg opp av seg selv. Det settes ofte dren i lommen, for å fjerne blod og væske fra sårområdet under brystet som fjernes vanligvis 1-2 dager etter operasjon.
The final result can only be assessed after 3-6 months. Follow-up checks are arranged with the clinic; the day after surgery, after 1-2 weeks and 3-6 months, as well as a final check-up after a year. You must call to arrange the check-ups yourself, or make an appointment with the clinic on the day of surgery.
Bleeding and infection are uncommon and in very few cases require reoperation and/or antibiotic treatment. Anesthesia complications, such as allergic reactions to anesthetics, are extremely rare.
Breast augmentation does not in principle provide grounds for sick leave, but if you have a physical job, we recommend that you take this into account in the first few days after surgery.
Physical exercise and hard physical work should be avoided for about 4 weeks.
The scars are often red, itchy and hard at first, but over time they will fade and soften. The scars are, however, hidden by a bra and will not be visible in low-cut dresses. In rare cases, the scars can become wide and clearly visible, this depends on the woman's own inherited skin characteristics. We therefore recommend that you tape the scars for a couple of months after a breast augmentation. Plaster tape is available at pharmacies.
If correction is necessary due to complications or unsatisfactory results, this will be done free of charge within one year, provided there is a medical indication and a clear improvement potential. This is our guarantee period.

In Norway, it is not allowed to show before and after pictures of silicone from cosmetic procedures in marketing or on the clinic's website. However, we can show you beautiful pictures from previous patients when you come for a consultation with the surgeon. This can be very helpful when we discuss the size of the implant, and it can make it easier for you to imagine an expected result.
We can also offer a preliminary examination with a 3D imaging system. We take pictures of your body and can use image simulation to show you what an expected result will look like with different sizes and implant types.
